About the Role

We are seeking a highly skilled and motivated Nurse Consultant to join our Berkshire Urgent Community Response (UCR) and Virtual Frailty Ward (VFW) teams. This exciting opportunity allows you to apply your clinical expertise, strategic leadership, and passion for improving care within a forward-thinking service. This rotational post covers all four UCR VFW teams, enabling close collaboration with a wide range of professionals.

Key Responsibilities

As a Nurse Consultant, you will lead advanced, autonomous clinical care for older adults living with frailty, including assessment, prescribing, and care planning. You will maintain expert clinical skills while providing direct patient care for at least 50% of your time. The role requires you to support safe, evidence-based practice across the UCR VFW service and promote patient-centered care in partnership with families and carers.

Strategic Leadership

You will act as a senior clinical leader, offering expert advice, driving quality improvement, and promoting patient safety. The position requires you to represent the service in internal and external reviews, legal proceedings, and trust-wide initiatives. You will provide compassionate leadership, coaching, and mentorship to support team development and workforce sustainability.

Education and Training

A key aspect of this role is creating a learning culture that supports professional development for nurses and students. You will collaborate with education providers to develop and deliver training on frailty, ageing well, and clinical excellence. The position involves promoting evidence-based practice and enhancing the clinical competence of UCR VFW staff.

Person Specification – Essential Requirements

Education and Qualifications: Must be a Registered Nurse with the NMC, possess a Masters or other higher degree, and have a teaching qualification with recent experience of delivering training/seminars. Advanced History Taking and Physical Examination with Clinical Reasoning qualification is required, along with Non-medical prescribing and Leadership/Management qualifications.

Professional Development

Applicants must demonstrate evidence of continuing professional development. The role requires substantial experience in older adult and urgent care, with at least six years post-registration at senior nurse/management level and a minimum of three years practicing at an advanced clinical level with older people with complex needs.

Experience Requirements

Essential experience includes extensive team management, data analysis and reporting, senior leadership, audit and research experience, and working with external partners. You must have experience in policy and protocol development, engaging with senior managers, presenting with professional networks, and implementing major change initiatives.

Additional Requirements

Applicants must hold a UK Driving License, use a car daily, and be able to travel efficiently between multiple sites. The role requires advanced and effective leadership/communication skills, both written and verbal. You must have the ability to write for publication and demonstrate experience in managing budgets and budget setting.
