Job Summary

The Macmillan Lead Palliative Care Clinical Nurse Specialist uses expert levels of clinical decision making through specialist assessment and interpretation of clinical information to support patients and carers who have complex and/or changing physical symptoms, emotional or practical needs. The successful candidate will have overall responsibility for the care of a caseload of patients requiring specialist advice, accept referrals from other healthcare professionals, and work unsupervised.

Main Duties and Responsibilities

The Palliative Care team contributes to the delivery of outstanding patient care by providing a patient-centred, evidence-based specialist service for patients with Palliative and End of Life care needs and those important to them at Chesterfield Royal Hospital. The post holder will provide leadership, purpose and vision as the service lead for one aspect of the Palliative Care service.

Key Responsibilities Include:

Developing and supporting a programme of education that enables other team members to be competent in the delivery of all aspects of the Palliative Care service. Acting as a resource for all those involved in the care and management of patients who have Palliative Care needs. Promoting an innovative and progressive attitude to the continual improvement of patient care through both formal and informal educational strategies.

Service Development

Leading on service development and the implementation of new ways of working that improve the quality of patient care, including providing a visual presence in ED/EMU, admission avoidance, innovation in delivery of education, and sustainable 7-day services. Participating in diagnostic MDT meetings and coordinating patient care in partnership with the MDT coordinator.

Essential Qualifications and Training

Essential Requirements:
– 1st level degree or equivalent experience
– 1st level RGN – Non-medical nurse prescriber qualification
– Advance communication course/qualification
– Teaching and assessing qualification
– Significant post registration experience including in clinical speciality
