Job Summary
The successful candidate will hold a fixed-term consultant position in Geriatric Medicine at London North West University Healthcare NHS Trust. Our collective goal is to provide excellent care for our frail older patients both in hospital and back home. We aim to treat acute illness appropriately, enhance quality of life, reduce disability, recognize when life is nearing its end, and provide individualized person-centered care in the right setting at the right time.
Main Duties and Responsibilities
As a Locum Consultant in Medicine for Older People, you will provide comprehensive inpatient care for patients on our Medicine for Older People wards at Ealing Hospital. You will work in our established frailty unit, focusing on comprehensive geriatric assessment for older people presenting to the Emergency Department with frailty syndromes not requiring admission. The role also includes contributing to outpatient care in clinics, including movement disorder and general rapid access clinics for people with frailty syndromes.
Clinical Governance and Teaching
You will work with the clinical and managerial team to support clinical governance, including contribution to morbidity and mortality meetings, quality improvement projects, and clinical audit. The position requires active participation in undergraduate and postgraduate teaching, training, and supervision. You will also contribute to service development where relevant and liaise with nursing staff, healthcare assistants, therapists, and other members of the multidisciplinary team.
Acute Frailty Service
Our established Acute Frailty Service comprises a Multidisciplinary Team based in ED, CDU and Ealing Acute Care Unit, providing rapid and responsive Comprehensive Geriatric Assessment to older patients with frailty at the front door. This service creates individualized plans for patients with the aim to avoid admission if possible or reduce length of stay if admission is required. You will have the opportunity to lead the Frailty MDT, currently consisting of 2 Frailty Practitioners, one frailty nurse, one frailty OT, a frailty pharmacist and an SHO.
OPRAC and Sub-specialty Interests
The Older Persons Rapid Assessment Clinic (OPRAC) is a well-established service providing Comprehensive Geriatric Assessment in an outpatient setting with a mix of virtual and face-to-face appointments. OPRAC is now combined with Ealing Acute Care Unit so that older patients with frailty can access same day emergency care (SDEC), avoiding unnecessary admissions. Development of any special interests is encouraged and actively supported, such as Continence, Falls, Movement Disorders, Surgical liaison including Orthogeriatrics, and Community involvement including potential participation in the Ealing Frailty Virtual Ward.
On-Call Commitments
The position includes general medical on-call commitment of approximately two days per month and 8 weekend days per year. A typical medical take is 30-40 patients per 24 hours. This attracts a 1% on-call supplement and time off in lieu during the week. Consultants are on call from home overnight, providing a good work-life balance.
Administrative Support and Facilities
The appointee will be given appropriate shared office accommodation with provision of personal computer equipment (with PAS, email and internet access), telephone and secretarial support. Qualifications and Experience Requirements
Essential Qualifications
Applicants must possess MRCP and clinical training and experience equivalent to that required for gaining UK CCST in General Medicine and Geriatric Medicine. Entry on the Specialist Register in Geriatric Medicine and General Medicine via CCT (within 6 months of interview), CESR or European Community Rights is essential. The department is strongly supportive of any candidate who wishes to pursue portfolio pathway training with a view to gaining a Certificate of Eligibility for Specialist Registration (CESR).
Essential Experience and Knowledge
Candidates must demonstrate experience in the management of older people and ability to take full and independent responsibility for patient care. IT skills and computer literacy are essential, as is the ability to speak and write English to the appropriate standard necessary to fulfil the job requirements. For international applicants, we recognize the value of diverse medical experiences and welcome applications from those with equivalent international qualifications and experience.
Desirable Qualifications
While not essential, NHS experience within the last 3 years is desirable. Additional desirable qualifications include MD, PhD or equivalent, and evidence of ongoing interest and achievement in research. Personal Skills and Attributes
We seek candidates with energy, enthusiasm, and the ability to work under pressure. Excellent communication skills with colleagues, patients, relatives, GPs, nurses and other staff and agencies are essential. Understanding the principles of clinical governance and its implications, leadership skills, knowledge of finance/budgets, ability to organize and prioritize workload effectively, and ability to plan strategically are all required attributes.
Teaching and Management Experience
Experience of teaching and training undergraduates, postgraduates and junior medical staff is essential. Evidence of service audit or change management is desirable. For international candidates, we recognize that teaching experience may come in various forms and welcome diverse educational backgrounds.
