Job Overview
Worcester City PCN has an exciting opportunity for an experienced clinical pharmacist to join our established multidisciplinary care home team. This is a unique chance to work with vulnerable elderly patients across care homes in our Primary Care Network. We are seeking someone to take responsibility for leading all pharmacy and medication-related aspects of the Enhanced Health in Care Homes Network Direct Enhanced Service Specification.
About the Position
The Frailty Clinical Pharmacist will work closely with a multidisciplinary team comprising GPs, Advanced Nurse Practitioners, Pharmacists, Pharmacy Technicians, Paramedics, Physician Associates, Care Coordinators, and other local stakeholders. You will play a vital role in delivering high-quality, efficient pharmacy support to care home residents, ensuring optimal medication management and improving patient outcomes.
Location
This position is based at Henwick Halt Medical Centre, located at 1 Ingles Drive, Worcester, WR2 5HL. The successful candidate will be required to visit various care homes across the Worcester City PCN area as part of their role.
Employment Details
This is a permanent position with flexible working arrangements. The employer offers both full-time and part-time options, with hours negotiable based on candidate requirements. The salary is dependent on experience and will be discussed during the interview process.
Key Responsibilities
The successful candidate will lead, deliver, and evaluate a specialist care home pharmacy service by providing in-depth, patient-focused Structured Medication Reviews (SMRs) to care home residents. You will support care home visits and ward rounds as required, promoting the safe, high-quality, and cost-effective use of medicines in care homes.
Core Duties
- Provide specialist pharmaceutical advice in complex situations to healthcare professionals across the PCN and care home staff
- Communicate highly complex medicines-related information with residents and/or their carers who may have complex care and communication needs
- Work with the care home team to develop and coordinate discharge pathways for care home residents
- Contribute to reductions in medicine-related hospital admissions and readmissions
- Ensure prescribing and administration of medicines follows appropriate legislation and health and safety policies
Qualifications and Training
The ideal candidate will hold a Master’s degree in Pharmacy (MPharm) or equivalent qualification and be registered with the General Pharmaceutical Council. A minimum of 2 years postgraduate experience in pharmacy is essential, as demonstrated within a practice portfolio.
Desired Qualifications
While not essential, candidates with specialist knowledge acquired through post-graduate diploma level or equivalent training/experience are highly desirable. Additional desirable qualifications include Independent Prescriber qualification and membership of the Royal Pharmaceutical Society (RPS).
Essential Experience
Candidates must have Enhanced Disclosure and Barring Service (DBS) clearance. The role requires self-motivation, adaptability, and safeguarding training for both adults and children at level three. Applicants must also have current immunization status, basic life support training, and a full UK driving license.
Professional Skills
The successful candidate will demonstrate excellent written and verbal communication skills, with the ability to communicate complex and sensitive information in an understandable form to various audiences. You must be able to plan, manage, monitor, advise, and review general medicine optimization issues for long-term conditions.
Technical Requirements
Applicants should possess good IT skills and be able to obtain and analyze complex technical information. The role requires the ability to work under pressure, meet deadlines, produce timely reports, and gain acceptance for recommendations even when facing significant barriers.
Clinical Knowledge
Candidates must demonstrate in-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. An appreciation of the nature of GPS and general practices, along with an understanding of primary care prescribing concepts, is essential.
