About the Role
Halton OOH District Nursing service is seeking committed individuals to join their team as community staff nurses covering the night service. This is an excellent opportunity for qualified nurses to develop their career within the NHS while providing essential healthcare to local communities.
The district nursing team delivers comprehensive care to housebound patients in their homes and residential care settings. We provide treatment room services for ambulant patients and utilize a wide range of clinical skills to meet diverse patient needs.
Key Responsibilities
The successful candidate will work under the direction of the District Nursing Sister, delivering essential healthcare services including:
- Administration of IV medication and comprehensive medicines management
- Implementation of health promotion initiatives
- Delivery of end of life and supportive care
- Conduction of holistic patient assessments
- Application of patient-centered care approaches
- Management of complex wounds
- Delivery of specialized catheter care
Duties and Responsibilities
The successful candidate will perform holistic assessments of patient nursing needs, plan, implement and evaluate care delivery according to changing health requirements. You will ensure practice is evidence-based through undertaking audit and critically appraising literature to maintain high standards of care.
Effective liaison with the District Nurse team leader is crucial to ensure information sharing regarding patient care, care plan modifications, continuity of care, and complex care package reviews. You will also contribute to mentoring, teaching and assessing other healthcare professionals.
Person Specification
Essential Qualifications
- Registered First Level Nurse (RGN)
- NMC registration as an adult nurse
- Ability to perform complex assessments and care planning
Desirable Qualifications
- V100/V150 nurse prescriber qualification
- Teaching and Assessing qualification
- Supra pubic catheterisation skills
- Tissue viability qualification
- Leg Ulcer management certification
- IV Therapy certification
- Chronic disease management experience
Essential Skills and Experience
- Relevant nursing experience in community settings
- Ability to travel independently across the Trust
- Flexibility to work according to service demands
- Competence in male and female catheterisation
- Experience in palliative care delivery
- Effective communication of sensitive information
- Understanding of professional and NHS policies
