Job Summary
We are looking for passionate and skilled Band 5 Community Nurses to join our Thanet Community Nursing Teams, to be based at our brand new £10million state-of-the-art Thanet Health Hub. Our new hub directly supports the Government’s new 10-year plan, that places community care at the heart of our health system. This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention.
About Thanet Health Hub
Opened in Autumn 2025, our new Thanet Health Hub consists of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK. With the forward vision of the NHS being in community-based care, there truly has never been a better time to join us.
Main Duties
As Community Nurses we support the nursing needs of house bound patients living in and around the areas of Thanet. We will enable and empower patients to self-care where possible so that patients can stay safe and well at home. We provide valuable care, supporting people to manage their long-term conditions and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they are admitted.
Care Services Provided
As a service, we may provide wound management, palliative and end of life care and symptom control and catheter care. We also take blood and administer injectable medication. Our aim is to ensure that our patients have access to the care that they need, at the time they need it. You’ll play a vital role in our multidisciplinary services; managing patients with complex needs in the community.

You will have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to great supervision and a clinical lead to make sure you’re on the right track. Essential Qualifications
You will need a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing. A full, valid UK driving licence, or a full, valid driving licence issued by an EU country (not exchanged from a non-EU country licence) and the ability to travel independently are essential for this role.
Desirable Qualifications
While not mandatory, a mentoring qualification equivalent would be advantageous for this role. We support professional development and can provide opportunities to gain additional qualifications while working with us.
Essential Experience
Recent experience of healthcare gained through education, work experience or previous employment is essential. This role is ideal for a newly qualified graduate nurse who is looking to start their career in the NHS.
Essential Knowledge
You must have knowledge of up-to-date clinical practice by using up-to-date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. Having an awareness of developments within the NHS would be an advantage.
