This permanent position offers a competitive salary ranging from £55,000 to £65,000 per year, with flexible working arrangements including full-time, part-time, job share, and compressed hours options.

Job Summary

We are currently seeking an Independent Prescribing Pharmacist to join our friendly multidisciplinary team. This is a fantastic opportunity to contribute directly to patient care, leading on safe and effective prescribing, managing chronic conditions, and supporting medication optimisation across our practices. The ideal candidate will be a registered pharmacist with the General Pharmaceutical Council and hold an Independent Prescriber qualification.

Key Responsibilities

As a Clinical Pharmacist with Reach Healthcare, you will conduct patient consultations and prescribe within your clinical competence. You will perform medication reviews and support deprescribing initiatives, working closely with GPs, AHPs, nurses, and other healthcare professionals to enhance care pathways. The role involves taking a proactive approach to managing long-term conditions and promoting antimicrobial stewardship.

Primary Duties Include:

  • Undertaking patient consultations, assessments, and prescribing within your clinical competence
  • Providing expert advice on medications, ensuring safe and effective prescribing in line with local and national guidelines

Person Specification

Essential Requirements:

  • Registered Pharmacist with the General Pharmaceutical Council
  • Independent Prescriber qualification

  • Ability to work autonomously as well as part of a multidisciplinary team
  • Excellent communication and patient consultation skills

Desirable Skills & Experience:

  • Experience in managing long-term conditions
  • li>Knowledge of GP systems such as EMIS or SystmOne

  • Experience in conducting medication reviews and structured prescribing interventions
