Location and Working Pattern

This position is based at the Jean Bishop Integrated Care Centre, located at 63-69 David Lister Drive, Hull, HU9 2BL. The role offers flexible working arrangements with the possibility of part-time hours (2-3 days per week) alongside a full-time position. Salary and Benefits

We offer a competitive salary ranging from £55,690 to £62,682 per annum, paid pro-rata for part-time positions. This is a Band 8a position under the Agenda for Change pay scheme. Key Responsibilities

The Clinical Pharmacist will provide pharmaceutical expertise within our Frailty Service, working alongside doctors, nurses, and therapists to deliver comprehensive geriatric assessments. You will be responsible for medication reviews, prescribing support, and ensuring the safe and effective use of medicines across multiple settings including the Jean Bishop Integrated Care Centre, care homes, community bedded units, and our virtual ward service.

Core Duties

Your primary responsibilities will include conducting structured medication reviews, providing pharmaceutical advice to the multidisciplinary team, and contributing to care planning for frail older adults. You will be expected to apply clinical knowledge appropriately in various practice settings and patient groups, with a particular focus on the unique medication needs of the elderly population.

Service Development

We are looking for someone interested in contributing to service development and improvement within the Frailty team. The successful candidate will have opportunities to participate in projects and initiatives that enhance medication management for frail patients across our integrated care services.

Person Specification

Essential Qualifications

To be considered for this role, you must hold a Masters degree in Pharmacy (MPharm) or equivalent pre-2000 qualification, plus one year’s pre-registration training. You must also be registered with the General Pharmaceutical Council (GPhC) and have evidence of Continuing Professional Development.

Essential Knowledge

You should demonstrate the ability to apply clinical knowledge appropriately in various practice settings or patient groups. This includes understanding current national policies and legislation relating to medicines, having thorough knowledge of primary care and medicines management issues, and possessing good analytical and decision-making skills around medicines information.

Essential Experience

We are seeking candidates with significant post-registration experience, including the ability to successfully implement change within a multidisciplinary team. Experience of influencing others through evidence-based medicine and clinical experience of conducting medication reviews are also essential requirements for this role.

Desirable Qualifications

While not essential, having an independent prescribing qualification would be advantageous. Membership of a professional organisation such as the Royal Pharmaceutical Society (RPS) is also desirable.

Desirable Knowledge

Familiarity with SystmOne and other GP computer systems would be beneficial, as would experience of conducting evidence-based reviews of literature and/or journal articles.

Requirements

You must have an independent means of transport to travel around the geographic area our services operate across. The ability to work flexibly to meet the needs of the organisation is also essential. All applicants must have current UK professional registration as a pharmacist.
